Job Summary/Purpose: The Panel Manager, as a member of the Patient Centered Medical Home (PCMH) Care Management Team in University Internal Medicine, will proactively coordinate with the patient and care team to achieve timely patient access to needed comprehensive and continuity of care services as well as specialized case management services provided to highly complex patients. Panel management is an organized, population-based, multidisciplinary team approach, in which the care team jointly plans and manages the care of patients assigned to the practice, including and especially those who do not regularly interact with the practice. As an extension of the patient-centered medical home (PCMH), the Panel Manager identifies patients in need of preventive and chronic disease management services, performs proactive outreach to those patients through multiple methods, and provides data to facilitate accurate tracking of improvements in continuity of care and population health metrics.

Required Minimum Training and Education: 

High School diploma and 2+ years of experience in healthcare setting required, preferably with patient interaction and/or schedule coordination and some level of demonstrated autonomy in previous roles. Proficient in Microsoft Word and Excel. Epic EMR experience and working knowledge of medical terminology would be helpful.

Job Duties and Responsibilities:  

  • Facilitate access needs of patients for current visits.  Regularly review information/reports for patients attributed to the medical teams and empaneled to the PCP.
  • Facilitate access needs of patients without an appointment (patient reminders, follow-up phone calls, labs, coordination of services, referrals).
  • Proactively outreach to patients to schedule appointments for preventative care services for health maintenance, labs, and follow-up appointments.
  • Facilitate the development and implementation of strategies to reach target populations.
  • Accesses practice schedule to identify upcoming appointments for patients’ continuity and gaps in care
  • Review and actively manage the assigned panel lists quarterly for appropriate size (both active and non-active patients) for new or lost to follow-up patients, patients diagnosed with chronic illnesses, or those who have abnormal/overdue labs
  • Open and manage new schedules and the templates of the providers assigned for panel management through coordination with and approval of Clinic Director and Division Director.
  • Track scheduling to ensure optimal access based on each appointment type. Facilitate same-day access to assigned PCP/team for patients with urgent issues.
  • Regularly use registry to coordinate care for current and future visits as needed.
  • Actively outreach to a registry of patients, using Epic EHR to follow patients based on panels belonging to individual primary care providers.
  • Coordinate with other members of the Care Management Team to ensure patients with complex medical and/or psychosocial problems are referred and tracked
  • Provide reports on various essential measures related to care coordination to gauge PCMH performance.
  • Prepare reports in collaboration with Section of Health Systems Research & Policy (HSRP) data analysts to track patient visits (goals) and monitor impact of panel management to demonstrate value and opportunities for outreach activities.
  • Work with HSRP data analysts to provide detailed and concise information and reporting to the Clinic Director and Division Director for strategic planning and organizational direction.  Assist with care coordination.  Engage community resources when necessary. Function as a patient navigator.
  • Coordinate patient access to ongoing medical care through care management and follow-up
  • Participate in trainings and care coordination meetings as assigned.
  • Follow standard protocols to facilitate referrals to internal and external entities.
  • Triage to Case Manager, PharmD, social worker as per protocol.
  • Understand HIPAA regulations and comply with organizational policies surrounding access and use of personal health information.

Physical Requirements: Continuous requirements are to perform job functions while standing, walking and sitting. Ability to bend at the waist, kneel, climb stairs, reach in all directions, fully use both hands and legs, possess good finger dexterity, perform repetitive motions with hands/wrists/elbows and shoulders, reach in all directions. Maintain 20/40 vision corrected, see and recognize objects close at hand and at a distance, work in a latex safe environment and work indoors.  Frequently lift and/or carry objects weighing 20 lbs (+/-) unassisted. Lift from 36” to overhead 15 lbs.  Infrequently work in dusty areas and confined/cramped spaces.
