Job Description SummaryManages and monitors Pediatric work queues in Epic/or other processes by conducting and completing the MLP standard of care including triage, screening, legal intake, and follow-up.
Job DescriptionCollaborates with physicians and multidisciplinary team members to develop a plan of care for each assigned patient from admission through discharge. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations. Assesses and responds to patient/family needs by coordinating efforts of other team members. Identifies and resolves barriers that hinder effective patient care.
Additional Job Description
Master's degree in SW from an accredited school of Social Work and three years work experience as a clinical Social Work required. Prior leadership experience preferred. One year of case management and/or utilization review work experience preferred. Case Management certification preferred. Licensure as a Social Worker by the state of South Carolina or compact state required.
