Job Description Summary
Manages and monitors Pediatric work queues in Epic/or other processes by conducting and completing the MLP standard of care including triage, screening, legal intake, and follow-up. Assists the MLP Legal Director and MLP Paralegal with case management and monitoring of current MLP cases. Provides MLP program coordination and communication, including status updates of MLP cases, between attorneys, MLP staff, and referring providers. Assists physicians, system administrative officer, leadership with presentations and outreach efforts within the three partnering organizations, in the community, and with all student interns and volunteers. Provides MLP program coordination and communication, including status updates of MLP cases, between attorneys and referring physicians and clinicians. Coordinates placement of MSW interns into the MLP; including screening and interviewing applicants, and supervising all interactions with staff and facilities. Assists providers and system administrative officer with placement and training of interns. Coordinates with CHW leadership, MUSC and community resources for referral or placement of non-legal issues identified in the MLP screening process. Assures collection of appropriate records to support grant funding and reporting, as well as any and all required MUSC, legal/attorney compliance and/or reporting. Assists with development of evaluation tools and assures collection of appropriate data for program evaluation and outcomes. Completes performance review process with supervisor to understand job performance and expectations; follows all MUSC safety procedures.
Job Description
Collaborates with physicians and multidisciplinary team members to develop a plan of care for each assigned patient from admission through discharge. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations. Assesses and responds to patient/family needs by coordinating efforts of other team members. Identifies and resolves barriers that hinder effective patient care.
Additional Job Description
Master's degree in SW from an accredited school of Social Work and three years work experience as a clinical Social Work required. Prior leadership experience preferred. One year of case management and/or utilization review work experience preferred. Case Management certification preferred. Licensure as a Social Worker by the state of South Carolina or compact state required.
