Job Summary/Purpose: The Home Health Social Worker provides psychosocial assessments, diagnosis and treatment to patients in their place of residences and prescribed by a physician, to assist the patient/family member(s)/caregiver(s) develop effective coping skills in response to the patient's diagnosis, treatment and/or life situation.
Minimum Training and Education: Master's degree in a social work or social welfare program accredited by the Council on Social Work Education and three years professional social work experience.
Required Licensure, Certifications, Registrations: MSW licensure by the South Carolina Board of Social Work Examiners. Certification in case management preferred.
