The nurse navigator will work directly with the provider to ensure safe and timely care for patients requiring intrathecal pump management. The navigator will facilitate referrals from Neurology/Neurosurgery to PM&R for patients with spasticity who have an implanted intrathecal baclofen pump. He/she will monitor patients over time and ensure they are making it to their visits for refills; communicate with families regarding hospitalizations and ER visits and with providers in other hospital systems as well as our own ED and admitting services when these situations arise; drive monthly operations meetings regarding these patients and to manage patients who will be getting intrathecal baclofen trials and implants with coordination of care with Neurosurgery for the implant process. The nurse navigator will assess Social Determinants of Health and help patients and families to make connections with appropriate resources and will be crucial to ensuring patients have the resources in place to ensure they are able to get to their appointments and that a constant level of support is available to provide any education or nursing support as needed to ensure patients and families are able to manage these devices at home between visits.
Additional Job Description
Bachelor's degree in Nursing from an accredited school of nursing and a minimum of three years related clinical nursing experience required. Master of Nursing (MSN) or Doctor of Nursing Practice (DNP) in lieu of BSN. Excellent written and verbal communications skills as well as mentoring/coaching and organizational skills required. Demonstrated ability to develop, implement and evaluate effective patient and staff educational programs. Creative and resourceful in problem solving and exceptional critical thinking required. Licensure as a registered nurse by the South Carolina Board of Nursing or a compact state. Current American Heart Association (AHA) Basic Life Support (BLS) certification or American Red Cross BLS for Healthcare Providers certification is required.
