Position Overview
We are seeking an experienced and dynamic Registered Nurse (RN) Coordinator to support our Ambulatory Palliative Care clinic and pilot partnership focused on patient-centered care and clinical innovation. The RN Coordinator will serve as a leading point of contact for patients, triaging care needs, navigating clinical pathways, and collaborating with multidisciplinary teams to ensure the success of the program. This individual will play a vital role as both a leader and a liaison—bridging patients, providers, program leadership, and external partners.

Job Description
Key Responsibilities
Triage & Navigation: Assess patient needs, provide timely triage, and guide patients through appropriate clinical pathways.
Patient Interaction: Build strong relationships with patients, ensuring compassionate communication, education, and follow-up.
Clinical Coordination: Collaborate with physicians, advanced practice providers, and support staff to ensure smooth care transitions.
Program Outreach: Outreaching and enrolling (patients/families) or (new referrals) in program initiatives and coordinating new patient visits with the interdisciplinary team.
Program Leadership Liaison: Act as a central point of contact between frontline clinical operations, program leadership, and external partners.
Data Collection & Reporting: Gather, track, and analyze patient and program data to inform continuous improvement and meet reporting requirements.
Collaboration & Teamwork: Work closely within a high-performing, advanced clinical team, demonstrating professional accountability and respect.
Leadership & Initiative: Take ownership of workflows, proactively solve problems, and contribute to the development of best practices.
Organization & Process Management: Maintain highly organized systems for documentation, scheduling, and follow-up to support program efficiency.

Qualifications
Active Registered Nurse (RN) license in good standing.
Minimum 5 years of clinical nursing experience; experience in care coordination, case management, or nursing management preferred.
Strong background in clinical navigation, triage, and patient education.
Demonstrated ability to work within an advanced interdisciplinary team.
Proven experience with data collection, reporting, and outcome measurement.
Exceptional organizational skills and attention to detail.
Ability to communicate clearly and professionally with patients, colleagues, leadership, and external partners.
Self-starter with strong problem-solving skills and the ability to manage competing priorities in a hybrid work environment.
Proficiency with electronic health records (EHRs), data management tools, and MS Office Suite.
