Job Description
The Value‑Based Care Program Manager is responsible for the planning, execution, and ongoing management of value‑based care (VBC) initiatives, including quality, cost, and utilization management programs tied to alternative payment models. This role partners closely with clinical, financial, operational, and data analytics teams to ensure successful program implementation, performance optimization, and compliance with payer and regulatory requirements.
The ideal candidate brings strong program management, healthcare operations, and data‑driven decision‑making skills, with a deep understanding of value‑based reimbursement models and population health strategies.
KEY RESPONISIBILITIES:
· Serve as the local operational leader for all value‑based care initiatives
· Act as the primary liaison between system VBC leadership and local clinical/operational teams
· Translate system strategy into local workflows, processes, and performance plans
· Coordinate quality improvement, population health initiatives, and care‑management activities
· Monitor contract performance and drive targeted improvement efforts
· Standardized reporting, communication, and accountability across departments
Support financial optimization through improved quality scores and shared savings
