Job Summary/Purpose: The RN Coordinator reports to the Manager of the Cardiovascular Clinic. Under guidance of the Manager of the Cardiovascular Clinic for the H&V ICCE, the RN Coordinator functions as a member of the clinical ICCE team. This role serves as a Navigator for the CHF Program by managing the patient experience from new patient referral to appointment in our Advanced Heart Failure clinic. This includes abstracting charts, managing patient scheduling for tests/procedures/clinic appointments, and following a panel of CHF patients in conjunction with the CHF Providers. In this role, the RN will occasionally travel to satellite clinics (once a month) to help support the patients that are being cared for there, under the supervision of the provider. The RN Coordinator assumes accountability for patient care outcomes through the assimilation and application of evidence-based practice to design, implement, and evaluate patient plans of care in a virtual environment setting. The RN Coordinator serves as a clinical expert related to resource utilization, case (care) management, monitoring vitals and making decisions on summary reports. A high degree of professional competence and autonomy will be demonstrated in this position.
Scheduled Work Hours/Shift: Monday-Friday 8am-5pm, no nights or weekends!
Minimum Training and Education: Bachelor's degree in nursing from an accredited school of nursing and three years related nursing experience. Some positions require certification as a generalist in a related specialty area by the American Nurses Association (ANA).
Required Licensure, Certifications, Registrations: Licensure as a registered nurse by the South Carolina Board of Nursing or a compact state. Some positions require certification as a generalist in a related specialty area by the American Nurses Association (ANA). Current Basic Life Support (BLS) required, either a certification from an American Heart Association (AHA) BLS for Healthcare Providers (or AHA recognized equivalent) or an American Red Cross CPR/AED for Professional Rescuer and Healthcare Provider."
