Job Description Summary

The Dementia Care Navigator position will provide guidance and support to dementia patients and their caregivers within the newly formed MUSC Health GUIDE Program. The GUIDE Program is an 8-year model that will be run from July 1, 2024 to June 30, 2032 aimed at helping patients with dementia holistically.

This full-time, on-site position reports directly to the Lead Dementia Care Navigator and the Guide Program Manager. This position actively supports Guide patients and their families in navigating the complexities of the dementia related diagnosis, including the treatment process, available resources, education, crisis prevention, and care coordination.

Job Description

Dementia Care Navigator Job Duties: 

Clinical Evaluation 35%: 

  • Assess the health status of patients through history taking, identify normal, and abnormal findings. 

  • Help develop individualized care plans based on assessed needs, including medical management, 

  • Behavioral support strategies, and community resource utilization. 

  • Implement communication strategies for persons with dementia and their caregivers. 

  • Facilitate referrals by obtaining clinical information and ensure the information is properly processed though corresponding departments. 

Care Coordination and Monitoring 30%: 

  • Review upcoming appointments in Epic, including those for Telehealth clinics, to ensure necessary services are arranged before the appointment. 

  • Ensure all testing and referrals are complete and necessary items addressed with the provider. 

  • Evaluate all patient test results, such as imaging and lab work, with GUIDE providers and execute the care plans discussed. 

  • Coordinate with referring physicians' offices to meet the needs of both patients and physicians. 

  • Provide comprehensive education on dementia symptoms, disease progression, treatment options, coping strategies, and caregiver support to patients and families, resulting in increased understanding and improved outcomes. 

Communication and Collaboration 35%: 

  • Serve as primary clinical contact for GUIDE patients and their caregivers regarding scheduling of testing and appointments. 

  • Serve as the liaison between the GUIDE clinical team and referring physicians. 

  • Provide emotional support and counseling tailored to the patient's clinical situation. 

  • Communicate with external case management and social services to ensure patients have access to individualized community support services. 

  • Assist in the development and facilitation of patient and caregiver programming, including support groups, educational classes, and seminars 

MUSC GUIDE Program Dementia Care Nurse Navigator Position Description 

This is a fulltime position that will be performed during normal business hours Monday-Friday on location. Flexibility in hours and location (e.g., hybrid - work from home) to accommodate the candidates schedule may be possible if they do not interfere with GUIDE operations. 

Core Requirements

  • Licensed Master of Social Work preferred 

  • Experience working with Alzheimer's disease patients and their families is strongly preferred.  

  • Fluent in English 

  • Experience in medical practice and knowledge of medical terminology is required 

  • Experience with care coordination/case management is helpful 

  • Fluent in English 

  • Proficient in use of EPIC and Microsoft Office preferred 

  • Knowledge of CPT codes, medical billing, and diagnosis coding helpful 

Additional Job Description

Education: Master's degree in a social work or social welfare program accredited by the Council on Social Work Education. Credentials: MSW licensure by the South Carolina Board of Social Work Examiners.  Certification in case management preferred.
