Specific responsibilities include interviewing clients and/or families to assess the client's current diagnosis, coping strategies, cognitive strengths and deficits, situation strengths and weaknesses and mental status; developing and discussing proposed treatment plans with the client and detailing the recommendations and the reasoning for them; monitoring and documenting progress of clients and making service/treatment plan adjustments as necessary. Conducts community planning, needs assessment and evaluation concerning societal conditions which impact on social functioning.
Must ensure established agency procedures are followed and that clinical standards that are set for treatment planning, the delivery of individual case management services, group and psycho-educational data, discharge planning and referral services are met. Also responsible for planning, implementing and/or coordinating all treatment services delivered to clients. Collaboration and coordination with both internal and external resources are crucial to ensure successful client outcomes. Must be able to take suggestions from team supervisors.
Additional Job Description
Required Minimum Training and Education: Master’s Degree in Social Work from an accredited program.
Required Licensure, Certifications, Registrations: Licensed in South Carolina. LISW-CP licensure/status is required for outpatient clinical services.
Degree of Supervision: Limited supervision but operates within a clinical team, which includes supervision by a clinical director (MD or PhD).
Physical Requirements
Continuous requirements are to perform job functions while standing, walking and sitting. Ability to bend at the waist, kneel, climb stairs, reach in all directions, fully use both hands and legs, possess good finger dexterity, perform repetitive motions with hands/wrists/elbows and shoulders, reach in all directions. Maintain 20/40 vision corrected, see and recognize objects close at hand and at a distance, work in a latex safe environment and work indoors. Frequently lift and/or carry objects weighing 20 lbs (+/-) unassisted. Lift from 36” to overhead 15 lbs. Infrequently work in dusty areas and confined/cramped spaces.
