Job Description
Licensures/ Certifications: Registered Dietitian required. Certified diabetes educator preferred. Experience: Two (2) years experience in dietetic work setting preferred. At least one (1) year clinical experience setting preferred.
The following description of job responsibility and standards is intended to reflect the major responsibilities and duties of the job but is not intended to describe minor duties and other responsibilities as may be assigned. All are essential job functions according to ADA guidelines and are listed in order of importance.
1. Assess the nutritional status and provide individualized nutrition care plans and education from the analysis of medical records and/or patient face to face interview for Bariatric Surgery patients utilizing specialized knowledge and skills; provide sound advice to patients on healthy eating habits and evaluate treatments of disease through nutrition.
2. Document findings, actions and summary assessments, follow up and diet instructions and comprehension in Epic electronic medical record system.
3. Assist Bariatric Surgery staff and providers with discharge planning processes as it relates to nutritional care plans.
4. Educate and council patients of all ages and caregivers on modification of diet required for continued recovery.
5. Review and modifies the patient room service meal selections as needed; communicate with Food & Nutrition Departments and providers; document for follow up visits, snacks and supplement needs.
6. Provide information to the medical team, including doctors, nurses, physical therapists, OT’s, ST’s and pharmacists for the care of patients in the IP and OP setting.
7. Perform Bariatric Surgery outpatient consultations as ordered by providers regarding nutritional assessments, weight concerns, diabetes or other disease processes related to diets and diet planning; provide feedback/documentation to providers and/or caregivers.
8. Promote and coordinate healthy eating habits to prevent and treat illness through supervision of food preparation and service, dietary modifications and public education.
9. May conduct formal classroom instruction to regarding medical nutrition to patients, groups, staff and providers.
