Job Description Summary
The primary purpose of this position is to work with the outpatient parenteral antimicrobial therapy (OPAT) group (currently comprised of an infectious disease PharmD, RN, and APP) and telehealth ID (Tele-ID) group (currently comprised of infectious disease providers including physicians and APP). This position directly reports to the Tele-ID and OPAT director. The administrative coordinator will work with both infectious disease groups to help coordinate patient care through direct patient communication, communication between providers and care groups, and data entry.Entity

Job Description
Job Duties:
20% - The Administrative Coordinator will be expected to review/monitor patient lab studies (such as complete metabolic panel and complete cell count) while the patient is receiving intravenous antimicrobials and triage results to appropriate groups or providers. These results will be retrieved through EPIC resulting (if an MUSC lab is used) and via fax (if a non-MUSC lab is used). The coordinator will also ensure all lab results are integrated into Epic for documentation purposes.
20% - The Administrative Coordinator, under the direction of the OPAT team, will communicate directly with patients via telephone and/or MyChart. The Administrative Coordinator will serve as the liaison for outpatient and inpatient tele-encounters between Tele-health and providers. This includes communicating with Telehealth staff on Teams, reaching providers if needed, and updating outpatient schedule with changes.
20% - The Administrative Coordinator will manage/organize the inpatient consultation schedule for up to 8 patients per day and 40 patients per week. Ensuring schedule is appropriate for specific provider and all slots are being filled for maximum coverage. The Administrative Coordinator will assist in confirming outpatient tele encounters by contacting the patient by phone or MyChart within 72 hours of their scheduled appointment.
15% - The Administrative Coordinator will assist in managing the OPAT patient list with the OPAT team. This will include adding names to existing patient lists within EPIC and entering patient data into a database (currently REDCap™). Upon transition to use of an EPIC OPAT Navigator tool, the coordinator will be expected to learn this technology to enter appropriate patient data into the system for monitoring purposes and quality improvement.
10% - The Administrative Coordinator will confirm that appropriate consultation documentation from the provider was filed into the patient’s electronic health record upon completion of the consult.
10% - The Administrative Coordinator will receive/return tele-related phone calls from patients or providers and triage messages to providers as appropriate through EPIC messaging.
5% - The Administrative Coordinator will assist in improving coordination of care for patients transitioning from the hospital to the ambulatory setting. This includes identifying the patient’s Home Health and Infusion Companies with contact person(s). This information will be documented in EPIC.

Minimum Requirements: A high school diploma and four years relevant experience in business management, public administration or administrative services; or a bachelor's degree and two years experience in business management, public administration or administrative services.
