Job Description Summary

This Human Service Coordinator I position will function as a Clinic Intake Coordinator/Case Manager for the Pediatric Collaborative Care (PCC) Program. The Clinic Intake Coordinator/Case Manager will provide individual evaluations of patients (via phone, remote or in-person) referred to the PPC to assess presenting symptoms, crime and trauma history, and other stressors in order to develop the best treatment plan and identify other resources and/or community referrals as needed. The Clinic Intake Coordinator/Case Manager will provide on-going intensive case management and coordination of services, along with brief interventions for victims of crime in the PPC clinic. This position includes comprehensive needs assessments, clinical case coordination, community-based services, coordination of services with local agencies who serve victims of crime, program evaluation, and other administrative duties. Victim services experience preferred.

Job Description

FLSA: Hourly

Job Duties:

  • 35% - Serve as a Clinic Intake Coordinator/Case Manager for the PPC Clinic and provide individual comprehensive needs assessments and initial screens for referred patients to assess presenting symptoms, crime and trauma history, basic needs, and victim-related needs. The position also involves provision of immediate crisis intervention and brief interventions, such as Psychological First Aid or Skills for Psychological Recovery, for patients on PPC waiting list.    
  • 25% - Provide ongoing care coordination, and referrals for additional services for victims of crime when needed by working closely with community partners.  Serve as main point of contact to residents, attendings and staff in PPC, as well as community partners regarding clinical services at the PPC. Provide community referrals for victims of crime referred to PPC, as well as serve as main point of contact for scheduling, communicating, and organizing victim needs to residents, attendings, other clinical staff, and trainees within the PPC. This will include scheduling appointments with other providers, making reminder calls for appointments, accessing resources for victims, victim advocacy, coordination of services with outside agencies also working with victims and their families (e.g., law enforcement, victims’ advocates, the judicial system, social services, medical providers), and arranging and providing transportation or childcare services.
  • 20% - Managing program tracking databases, checking for quality assurance, and evaluating program objectives in order to compile statistics for progress reports.
  • 10% - Assist in organization of the weekly staff meeting to include agenda setting, planning for training activities, and case review.  
  • 5% - Provide community presentations related to PPC, assist in preparation and participation in community outreach events.
  • 5% - Conduct other administrative duties as assigned by supervisor.

Preferred Training and Experience: Bachelors degree and one year of professional experience. Prior experience in human services or social services program is preferred. Experience working with victims of crime is helpful. Comfortable providing community-based outreach services. Experience working with multidisciplinary teams. Experience tracking service delivery. Knowledge of various software programs such as SPSS, Word, Excel, Access, PowerPoint a plus. Ability to communicate effectively, both orally and in writing. Ability to plan and organize work activities and prioritize task completion according to schedules and goals. Ability to exercise judgment and discretion. Strong work ethic is required. Occasional weekend and evening hours as needed to assist in coordination of community events. This is an in-person position, with some remote work allowable.
