Job Description Summary
The Lead Coordinator, Clinical Pharmacist/Medication Therapy Management (MTM) supervises Pharmacists, Pharmacist Interns, Technicians, and other staff of a clinical pharmacy outreach service that educates patients and their health care team to optimize medication related healthcare outcomes, ensure patient safety, recommend cost effective medication strategies, coordinate care with the member's care team and comply with CMS guidelines. These staff may also be involved in providing utilization management/prior authorization services as needed.Entity

Reviewing patients’ medication profiles and applying evidence-based medicine and national guidelines when creating recommendations to health care providers for optimizing the patients’ medication regimen.
Analyzing and reviewing medication lists for potential patient safety problems, including drug interactions, suboptimal medication regimens, etc.
Promoting clinically appropriate prescribing practices based on evidence-based medicine and national guidelines through various modalities (provider/plan profiling, member drug profile reviews, medication protocols/criteria, and case-by-case intervention).
Assisting in coordination of care for pharmacy-related issues between patients, providers, and other members of the patient’s interdisciplinary care team, including referrals to other MUSC or outside health care services. Following up with patients and providers to check on progress toward meeting drug treatment goals.

Assisting with clinical research projects to improve the MTM Program; Researching, developing, and implementing drug utilization and disease state management strategies and intervention techniques to deliver high-quality, cost-effective healthcare.
Working with other members of health care team (e.g., case managers, etc.) on patient-centered interdisciplinary teams; Serving as a drug information resource to Patients, Providers, and other members of heath care team.
Participating in the daily workload of the department, performing Pharmacist and other duties as needed.

Supervising the clinical team’s daily operations to ensure prompt and appropriate outreach and coordination of services, including MTM services for Medicare and Medicaid eligible members as well as Medication Adherence Programs for STAR, HEDIS and other targeted medications.
Ensuring that adequate staffing coverage is present at all times of operation.
Acting as liaison to internal and external customers to ensure prompt resolution of identified issues.
Assisting director and other Instructor/supervisor(s) with developing/updating policies and procedures and implementing changes to comply with state and federal regulations.
Assisting Director in the collection and tabulation of data for reporting purposes.
Interviewing job applicants and providing hiring recommendations to Director.
Providing staff training and coaching; helping to identify and provide for training needs, in collaboration with the Director.
Keeping Director apprised of operational issues, including staffing resources, program, and system needs.

Assists with the coordination of the Medication Therapy Management elective course, which may include developing course materials and evaluating student cases.
Serve as a preceptor for the Ambulatory Care 4th year APPE.
