Job Description

Clinical Oversight & Staff Supervision

  • Supervise floor nurses, charge nurses, and Certified Nursing Assistants (CNAs) to ensure adherence to facility policies and resident care standards.
  • Conduct daily resident rounds to evaluate the quality of care and report changes in resident condition to the DON or attending physician.
  • Assist the DON in tracking and ensuring all shifts are adequately staffed.

Regulatory Compliance & Quality Assurance

  • Assist in monitoring facility compliance with South Carolina Department of Public Health (DPH) standards and CMS regulations.
  • Help identify safety hazards and assist with Quality Assurance/Performance Improvement (QAPI) audits.
  • Participate in investigating incidents, falls, and grievances, implementing immediate corrective actions when necessary.

Administrative & Educational Duties

  • Assist with the orientation, training, and competency evaluation of new nursing staff.
  • Ensure medical records and resident care plans are accurately maintained.
  • Step in to provide direct patient care or medication administration during periods of high acuity or staff shortage.
  • Assist with the referral process for new admissions.
  • Other duties as assigned

Additional Job Description

Qualifications

  • Education: Graduate of an accredited practical nursing program.
  • Licensure: Active, unencumbered LPN license in the State of South Carolina & Basic Life Support (BLS)
  • Experience: Minimum of 3 to 5 years of clinical nursing experience, with at least 1 year in a long-term care or supervisory capacity preferred.
  • Knowledge: Deep understanding of SC DPH nursing home regulations, the SC Nurse Practice Act, and standard infection control protocols.

SC Regulatory Context

Under the South Carolina Nurse Practice Act (Section 40-33-42), an LPN is authorized to provide nursing care, participate in patient care planning, and supervise subordinate personnel (such as CNAs) under the supervision of an RN or physician. Furthermore, South Carolina DPH Regulation 61-17 for Nursing Homes requires the facility to designate a full-time RN as the Director of Nursing, while allowing facilities the operational flexibility to utilize qualified LPNs in supportive and delegated administrative/supervisory roles.
