Job Description

MUSC Health is looking for a Community Clinical Navigator to help shape the future of healthcare throughout the Catawba Division. This is an exciting opportunity for a clinically trained professional who thrives on building relationships, solving problems, connecting people to resources, and creating healthier communities.

In this highly visible role, you’ll be the clinical bridge between MUSC Health and the communities we serve—building trusted partnerships with senior living communities, active adult (55+) neighborhoods, employers, community organizations, healthcare providers, and civic leaders.

You’ll use your clinical expertise to identify community health needs, remove barriers to care, connect individuals and organizations with the right MUSC Health resources, and champion initiatives that promote healthy aging, wellness, prevention, healthcare access, and population health.

This is not a direct patient-care role. Instead, you’ll have the opportunity to influence health at the community level—bringing people, providers, programs, and resources together to create meaningful and measurable impact.

As the Community Clinical Navigator, you’ll help MUSC Health meet people where they are.

You’ll:

• Help individuals and organizations navigate MUSC Health services and resources.
• Identify barriers to care and develop creative solutions.
• Champion healthy aging, prevention, wellness, and quality of life for older adults.
• Bring health education, screenings, and wellness programming into the community.

• Use community health data and engagement trends to identify opportunities for improvement.
• Help turn community needs into strategic initiatives with measurable outcomes.
• Strengthen MUSC Health’s presence and impact throughout the Catawba Division.

What You’ll Do

Build Partnerships That Matter

You’ll serve as a trusted MUSC Health representative within your assigned communities, developing and sustaining relationships with senior living communities, active adult neighborhoods, employers, healthcare providers, community organizations, and civic leaders.

You’ll attend community meetings, lead and support educational events, participate in wellness initiatives, and identify new opportunities to expand MUSC Health’s reach and impact.

Make Healthcare Easier to Access

Your clinical knowledge will help you connect people and organizations to the right care, provider, service, wellness program, or community resource at the right time.

You’ll identify barriers to care, facilitate referrals and communication, promote preventive services and screenings, and help community partners navigate the healthcare resources available through MUSC Health.

Champion Healthy Aging

You’ll play a key role in initiatives designed to help older adults live healthier, more active, and more fulfilling lives.

You’ll collaborate with community partners to identify priorities, coordinate health education and wellness programs, support screenings and outreach events, and encourage participation in evidence-based preventive health and wellness programs.

Turn Data Into Action

This role is about more than building relationships—it’s about demonstrating results.

You’ll monitor participation, referral activity, partnership development, engagement trends, and program outcomes. You’ll use data, stakeholder feedback, and community health information to evaluate what’s working, identify opportunities, and recommend strategies that improve access and community impact.

Connect the Community to MUSC Health

You’ll collaborate with Business Health leadership, divisional leaders, physician practices, service lines, Marketing, and other MUSC teams to bring community priorities and MUSC resources together.

You’ll help create innovative approaches that expand access, strengthen partnerships, increase awareness of MUSC Health services, and advance OneMUSC strategic priorities.

You’ll help build the connections that make healthier communities possible.

What You Bring

We’re looking for a clinically trained healthcare professional with at least five years of relevant experience who is energized by community engagement, relationship building, healthcare navigation, and strategic problem-solving.

Additional Job Description

Minimum Qualifications

Education:
Bachelor’s degree in nursing, allied health, public health, healthcare administration, health sciences, community health, or another accredited clinical healthcare discipline.

Licensure/Certification:
Current South Carolina licensure or certification in an approved clinical healthcare discipline.

Examples include:

• Registered Nurse (RN)
• Athletic Trainer (ATC)
• Physical Therapist (PT/PTA)
• Occupational Therapist (OT/COTA)
• Respiratory Therapist (RRT)
• Registered Dietitian (RD)
• Licensed Practical Nurse (LPN)
• Paramedic
• Other licensed or certified clinical healthcare professionals approved by MUSC
• Current Basic Life Support (BLS) certification is preferred.

Experience:
At least five years of experience in one or more areas such as:

• Community health
• Population health
• Healthcare operations
• Healthcare access or navigation
• Care coordination
• Clinical outreach
• Wellness programming
• Occupational or employer health
• Senior services
• Healthcare administration
• Related healthcare environments

A valid driver’s license, reliable transportation, and the ability to travel frequently throughout the assigned service area are required.
