Job Description

In this fast-paced, high-impact role, you won’t just provide support—you’ll be a lifeline. You’ll help patients navigate emotional crises, guide families through uncertainty, and collaborate with medical teams to ensure truly holistic care.

What You’ll Do

  • Conduct meaningful assessments to understand patients’ social, emotional, and environmental needs
  • Provide immediate crisis intervention in high-stress, time-sensitive situations
  • Develop and coordinate personalized care and discharge plans
  • Support patients and families in coping with illness, trauma, and hospitalization
  • Act as an advocate, ensuring patients receive the resources and support they deserve
  • Collaborate closely with physicians, nurses, and interdisciplinary teams to deliver comprehensive care

Additional Job Description

Qualifications:

  • Master’s Degree in Social Work from an accredited program

  • 2–4 years of experience in healthcare, behavioral health, or related field

  • Licensed Social Worker (LMSW) in the state of South Carolina

  • Basic Life Support (BLS) Certification with American Heart Association required or must obtain upon hire
