Job Description Summary

The MUSC Maternal Health RN Care Coordinator is a grant-funded position that reports to the System
Manager for Population Health Care Coordination for the MUSC Population Health Program and works
closely with the Population Health team, Women’s and Maternal Health leadership teams, clinicians, and
staff. The RN Care Coordinator provides care coordination services for preconception health to
expecting mothers. The RN Care Coordinator promotes equitable care and timely access to treatment,
and supports reduction of avoidable acute care utilization and improved management of chronic
conditions, through the provision of collaborative care planning and health education, identification of
health-related social needs, coordinating and communicating with patients, family members, health care
providers and care team members, monitoring patient satisfaction with the Preconception and Maternal
Health experience, and connecting patients to follow-up services and community resources, and clinical
trials as medically appropriate.
This position works closely with the Maternal Health Nurse Navigators, Social Workers, Case Managers,
and schedulers, MUSC Women’s and Maternal Health Division physicians and staff, community
partners, and Population Health team members across the continuum of care for Maternal Health
patients.

Training and Education: Bachelor’s degree in Nursing preferred.  Two (2) years of related work experience as a registered nurse required. Two (2) years of Women’s or Maternal Health experience preferred. Ensure the provision of nursing care services in collaboration with other health professionals. Case Management experience preferred. Current knowledge base of procedures, techniques, equipment, technical advances and regulatory agencies and standards within the field of nursing. Current South Carolina licensure or licensure in a compact state as a registered nurse required. Current Basic Life Support (BLS) required, either a certification from an American Heart Association (AHA) BLS for Healthcare Providers (or AHA recognized equivalent) or an American Red Cross CPR/AED for Professional Rescuer and Healthcare Provider.

Additional Job Description

Bachelor’s degree in nursing required and three years related nursing experience. Licensure as a registered nurse by the South Carolina Board of Nursing or a compact state. Some positions require certification as a generalist in a related specialty area by the American Nurses Association (ANA). Licensure as a registered nurse by the South Carolina Board of Nursing or a compact state. Current American Heart Association (AHA) Basic Life Support (BLS) certification or American Red Cross BLS for Healthcare Providers certification is required
