Job Description Summary

The Registered Dietitian (RD) and Certified Diabetes Care and Education Specialist (CDCES) serves as a vital clinical partner to individuals navigating Type 1, Type 2, gestational, and pre-diabetes. This role focuses on empowering patients through evidence-based behavioral coaching, tailored lifestyle modifications, and modern diabetes technology, rather than relying on restrictive dietary templates. The ultimate objective is to assist patients in achieving optimal glycemic control, increasing Time in Range (TIR), and improving overall quality of life within a collaborative, multidisciplinary medical environment. This role will support both inpatient and outpatients for those with diabetes including patients enrolled in SC MOMs (high-risk pregnant patients).

Job Description

Job Duties:

Clinical Assessment & Personalized Counseling

  • Conduct comprehensive nutritional assessments, evaluating patients' medical histories, lab values (A1C, lipid panels, eGFR), lifestyle habits, and socioeconomic factors.

  • Develop personalized, culturally sensitive Medical Nutrition Therapy (MNT) plans that align with the American Diabetes Association (ADA) standards of care.

  • Provide individual and group counseling on carbohydrate counting, glycemic index, macronutrient distribution, and weight management.

Diabetes Technology Integration

  • Train patients on utilizing Continuous Glucose Monitors (CGMs) and smart insulin pens/pumps.

  • Analyze ambulatory glucose profile (AGP) reports to identify glycemic patterns, glucose variability, and "Time in Range" (TIR).

  • Collaborate with endocrinologists and primary care providers to recommend adjustments to insulin or non-insulin medications based on glucose data.

Patient Education & Advocacy

  • Deliver formal Diabetes Self-Management Education and Support (DSMES) curriculum covering acute complications (hypoglycemia/hyperglycemia) and long-term risk reduction.

  • Empower patients with problem-solving skills for real-world scenarios (e.g., dining out, traveling, sick-day management, and exercise).

  • Address barriers to care, such as food insecurity or financial constraints regarding diabetes supplies.

Programmatic Support

  • Actively participate in meetings as assigned. Provide reports and updates on program development.

  • Develop and distribute educational materials for patients and providers.

  • Provide support for ADA and joint commission certification and documentation.

  • Conduct individual diabetes education visits as well we establish group diabetes classes.

Required Qualifications & Experience:

Education & Licensure

  • Bachelor’s or Master’s degree in Clinical Nutrition, Dietetics, or a related field.

  • Current Registered Dietitian (RD/RDN) credential via the Commission on Dietetic Registration.

  • Active state licensure/certification as a Dietitian (if applicable by state law).

Certifications & Experience

  • CDCES (Certified Diabetes Care and Education Specialist) credential is highly preferred (or eligible and actively tracking hours to sit for the exam within 1 year).

  • Minimum of 2–3 years of clinical experience, specifically working with diabetes populations in an outpatient, endocrine clinic, or community health setting.

  • Proficiency in reading and interpreting data from major CGM platforms (e.g., Dexcom, Abbott Libre) and pump software.

Additional Job Description

Bachelor’s degree in dietetics or related field of study from an American Dietetic Association approved program; Must be a Registered Dietitian; Certified Diabetic Educator preferred.
