The Nurse Practitioner or Physician Assistant serves as a key clinical team member supporting both Primary Care operations, Inbasketology, and Community Based programs. This role provides a blend of direct patient care, SCPA clinical coverage, virtual services, and inbasket management. Job Description
Schedule
Full‑time, rotating assignment across in‑clinic, SCPA, and virtual support roles.
Required Training and Education:
Licensed Nurse Practitioner or Physician Assistant (Required)
Current state licensure and national certification (Required)
Current American Heart Association (AHA) Basic Life Support (BLS) certification or American Red Cross BLS for Healthcare Providers certification is required
Minimum of 1–2 years clinical experience in primary care, urgent care, or related setting preferred
Strong clinical assessment, communication, and decision‑making skills
Ability to work independently and collaboratively within a multidisciplinary team
Comfort with virtual care platforms and electronic medical record workflows
Key Responsibilities
Provide direct in‑clinic patient care within Primary Care practices based on daily operational needs.
Deliver dedicated clinical coverage to our community partners during assigned hours.
Perform Inbasketology responsibilities, including protected daily time for inbox management and additional support throughout clinic hours.
Provide virtual care and remote support services across all Primary Care clinics.
Participate in care coordination, quality initiatives, and population health efforts.
Maintain collaborative relationships with physicians, APP colleagues, and care teams to optimize patient access and care delivery.
Support coverage continuity during planned and unplanned absences to ensure contractual staffing requirements are met

Physical Requirements
Continuous requirements are to perform job functions while standing, walking and sitting. Ability to bend at the waist, kneel, climb stairs, reach in all directions, fully use both hands and legs, possess good finger dexterity, perform repetitive motions with hands/wrists/elbows and shoulders, reach in all directions. Maintain 20/40 vision corrected, see and recognize objects close at hand and at a distance, work in a latex safe environment and work indoors. Frequently lift, lower, push and pull and/or carry objects weighing 50 lbs (+/-) unassisted, exert up to 50 lbs of force, lift from 36" to overhead 25 lbs. Infrequently work in dusty areas and confined/cramped spaces.
