Job Description
The Palliative RN Coordinator plans and delivers direct patient care utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation; and effectively interacts with patients, families, and other interdisciplinary team members while maintaining standards of professional nursing and clinical competency. This position systematically triages patients based upon clinical problems and educational needs, provides coordination of care for the patient, and is also a resource to clinic staff regarding the direct provision of care.
The Palliative RN Coordinator is responsible for identifying and coordinating patient/family care to support patients and families in home, skilled nursing facilities and other residential care facilities. They will coordinate and case manage using techniques that include person and family centered care planning, shared decision making, symptom management, and counseling. The Palliative RN Coordinator endeavors to utilize teaching, assessment, and intervention skills to provide symptom management and maximize the quality of life for the patients and families.
This individual will be an active member of the multidisciplinary team and collaborates in the development, execution, and evaluation of the multidisciplinary plan of care. This includes the establishment of goals and strategies for meeting the continuing care needs of the patient, family, and/or other care provider.
This position works with the team to assure that systems of care are in place for patients to move across the continuum of care. This includes development of patient care protocols, order sets, and systems that allow care to take place in an effective and efficient manner. This position also puts systems in place to evaluate the effectiveness of patient care.
Additional Job Description
Bachelor's degree in nursing from an accredited school of nursing and three years related nursing experience. Licensure as a registered nurse by the South Carolina Board of Nursing or a compact state. Current Basic Life Support (BLS) required, either a certification from an American Heart Association (AHA) BLS for Healthcare Providers (or AHA recognized equivalent) or an American Red Cross CPR/AED for Professional Rescuer and Healthcare Provider.
