Job Description Summary

The Midlands patient care coordinator reports to the Director of Health Champions. Under limited supervision, the Midlands patient care coordinator serves as a patient and family liaison for the midlands division. Works closely with other patient care coordinators, the patient access center and other health care team members in facilitating patient transitions of care and provides/plans for patient support services as needed. Facilitates and processes new patient referrals, utilizing advanced nursing skills in the assessment and planning of clinical care for identified patients. This position monitors data related to patient referrals and creates consistent reporting communication to send to partners along with MUSC Health Leaders.

Job Summary/Purpose: The Patient Care Coordinator for Regional Health Care System reports to the Director of Health Champions. Under limited supervision, the patient care coordinator serves as a patient and family liaison for the midlands division.  Works closely with other patient care coordinators, the patient access center and other health care team members in facilitating patient transitions of care and provides/plans for patient support services as needed. Facilitates and processes new patient referrals, utilizing advanced nursing skills in the assessment and planning of clinical care for identified patients. This position monitors data related to patient referrals and creates consistent reporting communication to send to partners along with MUSC Health Leaders. This position serves the Regional Health Network. This position is remote but will require travel to on-site regional facilities.

Minimum Training and Education: Bachelor's degree in nursing from an accredited school of nursing and three years related nursing experience. Some positions require certification as a generalist in a related specialty area by the American Nurses Association (ANA).

Required Licensure, Certifications, Registrations: Licensure as a registered nurse by the South Carolina Board of Nursing or a compact state. Some positions require certification as a generalist in a related specialty area by the American Nurses Association (ANA). Current Basic Life Support (BLS) required, either a certification from an American Heart Association (AHA) BLS for Healthcare Providers (or AHA recognized equivalent) or an American Red Cross CPR/AED for Professional Rescuer and Healthcare Provider.

Ability to maintain 20/40 vision, corrected, in one eye or with both eyes. (Continuous) Ability to see and recognize objects close at hand or at a distance.
