Job Description Summary

The Patient Care Coordinator (PCC) plays a crucial role in ensuring the smooth transition of patients into the MUSC system. Job Summary/Purpose:

The Patient Care Coordinator (PCC) plays a crucial role in ensuring the smooth transition of patients into the MUSC system. They collaborate with the multidisciplinary team to develop comprehensive care plans that align with the principles and practices of the nursing process. The PCC actively participates in programmatic changes and updates clinical practice to meet the evolving needs of patients, considering their age, developmental stage, and the preferences of their families.

As the primary patient advocate, the PCC coordinates the daily needs of the nursing and technology teams, ensuring that the patient remains at the forefront of all decisions. They serve as the point of contact for patients seen by MUSC VIR, providing seamless coordination of care after each appointment and supporting patients throughout the entire care continuum. The PCC also acts as a liaison between patients, families, caregivers, and the multidisciplinary care team, fostering open communication and collaboration.

Minimum Training and Education: Bachelor's degree in nursing from an accredited school of nursing and three years related nursing experience. Some positions require certification as a generalist in a related specialty area by the American Nurses Association (ANA).

Required Licensure, Certifications, Registrations: Licensure as a registered nurse by the South Carolina Board of Nursing or a compact state. Some positions require certification as a generalist in a related specialty area by the American Nurses Association (ANA). Current Basic Life Support (BLS) required, either a certification from an American Heart Association (AHA) BLS for Healthcare Providers (or AHA recognized equivalent) or an American Red Cross CPR/AED for Professional Rescuer and Healthcare Provider.

Ability to maintain 20/40 vision, corrected, in one eye or with both eyes. (Continuous) Ability to see and recognize objects close at hand or at a distance.
