Job Description Summary

The MUSC Health System Manager of Post Acute Care reports to the System Director of Inpatient Case Management, and works closely with the Case Management and Care Transitions, and Post-Acute partners. MUSC Health System Manager of Post Acute Care supports departmental, ICCE, and Medical Center goals.

Goals include the achievement of clinical and quality measures, operational efficiency, and fiscal targets, with a focus on reducing hospital readmissions and decreasing LOS. The position is responsible for providing leadership, coordination, and facilitation of all activities and services across the system regarding the care continuum.

Job Summary/Purpose:  The MUSC Health System Manager of Post Acute Care reports to the System Director of Inpatient Case Management, and works closely with the Case Management and Care Transitions, and Post-Acute partners. MUSC Health System Manager of Post Acute Care supports departmental, ICCE, and Medical Center goals.

Minimum Training and Education: Bachelor's degree in social work or Bachelors of Nursin (BSN) degree required. 2 years of social work experience in an acute care setting required or 2 years of experience in community health, complex adult/pediatric patient care, or case management. Must have 1 year in the clinical application of crisis and/or trauma counseling and interventions.

Required Licensure, Certifications, Registrations:  Licensure as a registered nurse or licensure as a clinical social worker in the state of South Carolina. QAPI Certified Professional (QCP) preferred.
