Job Description

Under the general direction of the Director of Transplant Operations, the Social Work Manager provides leadership, oversight, and strategic direction for transplant social work services across all solid organ transplant programs and living donation. This position is responsible for the management, supervision, and professional development of the transplant social work team, ensuring the delivery of high-quality psychosocial care throughout the transplant continuum. The Social Work Manager collaborates with multidisciplinary leaders to support program goals, optimize patient outcomes, promote equitable access to transplantation, and ensure adherence to organizational, regulatory, and accreditation requirements.
The Social Work Manager oversees clinical practice standards, workforce planning, quality improvement initiatives, operational workflows, and regulatory compliance related to transplant psychosocial services. Responsibilities include developing and implementing policies and procedures, monitoring performance metrics, facilitating interdisciplinary care coordination, managing complex psychosocial issues, and fostering partnerships with internal and external stakeholders. This position serves as a key resource for transplant teams, patients, families, and living donors while advancing evidence-based practice, staff engagement, and continuous process improvement across all transplant programs.

Additional Job Description

30% – Living Donor Social Worker
Provide psychosocial assessments, education, counseling, and ongoing support for individuals considering living kidney or liver donation.
Evaluate prospective living donors’ readiness for donation, support systems, understanding of the donation process, financial and practical concerns, and potential barriers to donation.
Advocate for the autonomy, well-being, and informed decision-making of living donors throughout the evaluation, selection, donation, and recovery process.
Conduct post-donation follow-up and provide ongoing psychosocial support as needed.
Connect living donors with available financial assistance programs and resources to reduce barriers to donation.
Participate in transplant selection committee meetings by presenting psychosocial assessments and contributing to multidisciplinary decisions regarding donor candidacy.
25% – Collaboration with the Living Donor Team
Collaborate with physicians, transplant managers, nurse coordinators, dietitians, pharmacists, and other interdisciplinary team members to coordinate comprehensive donor and patient care.
Participate in regular multidisciplinary meetings to discuss donor evaluations, candidacy, care planning, and outcomes.
Support communication and coordination among team members to ensure efficient and patient-centered care throughout the donor evaluation and transplant process.
Assist with program development, quality improvement initiatives, and regulatory compliance activities related to living donation.
25% – Support of Transplant Social Work Staff
Conduct monthly check-ins with each organ transplant program to support communication, identify program needs, and coordinate patient-care resources.
Provide consultation, guidance, and support to transplant social workers regarding complex psychosocial concerns, resource needs, and patient-care coordination.
Manage the American Kidney Fund (AKF) Health Insurance Premium Program (HIPP) and GMS processes, including coordination, documentation, follow-up, and communication with appropriate stakeholders.
Develop and maintain internal resource networks to support transplant patients, caregivers, and living donors.
Maintain accurate and timely documentation in accordance with transplant center policies, regulatory requirements, and social work standards of practice.
20% – Community Outreach and Resource Development
Develop and maintain relationships with community organizations, advocacy groups, healthcare partners, financial assistance programs, and other external stakeholders.
Conduct outreach and networking activities to increase awareness of available resources and improve access to supportive services for transplant patients and living donors.
Identify and establish new partnerships that enhance financial, psychosocial, transportation, insurance, and behavioral health resources for transplant recipients and living donors.
Represent the transplant program at community meetings, educational events, and collaborative initiatives to strengthen community engagement and resource availability.
Promote awareness of transplant-related services and support programs through community education and relationship-building efforts.


Education: Master’s Degree in Social Work (MSW) is required
Work Experience: 7 years progressive work experience and 2 years management experience
State licensure and/or professional certification requirement/s dependent on position. Current American Heart Association (AHA) Basic Life Support (BLS) certification or American Red Cross BLS for Healthcare Providers certification is required or an American Red Cross CPR/AED for Professional Rescuer and Healthcare Provider.
