Job Description Summary

The Ophthalmology Department is seeking a dedicated and compassionate person to join our multidisciplinary team and become an integral member of the
Storm Eye Institute family. This Patient navigator (PN) position will address the need for comprehensive care coordination for highly complex eye patients. The PN
will facilitate communication, provide education, and coordinate services to improve patient adherence and health-outcomes. Many patients face challenges such
as mobility issues, financial hardship, and language barriers, making it difficult to access care. By assisting with appointment scheduling, transportation, and remote
support through MyChart and check-ins, the PN will ensure patients receive timely and appropriate care. This role, filled by a social worker, LPN or certified eye
technician, aims to increase compliance with medications, treatment regimens, follow up appointments and surgeries while enhancing the patient experience.
This position will receive a competitive salary commensurate with experience. This is a full time, on-site position for which candidates must have the ability to
maintain flexible hours occasionally including evenings and weekends to accommodate working professional clients.

MUSC Minimum Education and Experience Requirements:

A high school diploma and two years relevant program experience. A bachelor's degree may be substituted for the required program experience.

Preferred Education and Experience:

Two years of clinical experience; one year must be Ophthalmology-specific.

Job Duties:

Clinical Evaluation 35%:
Assess the health status of patients through history taking, identify normal, and abnormal findings.
Help develop individualized care plans based on assessed needs, including medical management.
Behavioral support strategies, and community resource utilization.
Implement communication strategies for persons eye disease/blindness and their caregivers.
Facilitate referrals by obtaining clinical information and ensure the information is properly processed.

Communication and Collaboration 35%:
Serve as primary clinical contact for Ophthalmology patients enrolled in patient the navigation program and their caregivers regarding scheduling of testing and
appointments.
Serve as the liaison between the Storm Eye clinical team and referring physicians.
Provide emotional support and counseling tailored to the patient's clinical situation.
Communicate with external stakeholders to ensure patients have access to individualized community support services.

Care Coordination and Monitoring 30%:
Review upcoming appointments in Epic, including those for Telehealth clinics, to ensure necessary services are arranged before the appointment.
Ensure all testing and referrals are complete and necessary items addressed with the provider.
Evaluate all patient test results, such as imaging and lab work, with providers and execute the care plans discussed.
Coordinate with referring physicians' offices to meet the needs of both patients and physicians.
Provide comprehensive education on symptoms, disease progression, treatment options, coping strategies, and caregiver support to patients and families,
resulting in increased understanding and improved outcomes.
