Job Description Summary

The System Director, Post-Acute Care is responsible for developing and implementing strategies designed to enable the smooth and timely transition of patients from the hospital to lower levels of care. This includes the development/oversight of skilled nursing facility (SNF) preferred partnerships, management of Assisted Living Facility (ALF) relationships and responsibility for the evaluation, planning, and development of other post-acute programs (e.g. long-term beds, respite services, hospice, GIP, contract beds, etc.) that may be required to support the needs of patients discharged from MUSC Health hospitals. This role serves as the hospital’s primary contact for post-acute partners and is responsible for monitoring their performance and ensuring that desired outcomes are achieved. The System Director closely collaborates with members of the Case Management and Population Health leadership teams and works with a broad range of administrative and clinical stakeholders across MUSC, MUSCP and MCP.

Job Summary/Purpose

 The System Director, Post-Acute Care is responsible for developing and implementing strategies designed to enable the smooth and timely transition of patients from the hospital to lower levels of care. This role serves as the hospital’s primary contact for post-acute partners and is responsible for monitoring their performance and ensuring that desired outcomes are achieved. The System Director closely collaborates with members of the Case Management and Population Health leadership teams and works with a broad range of administrative and clinical stakeholders across MUSC, MUSCP and MCP. 

 

Goals for this position include the achievement of clinical and quality measures, operational efficiency, and fiscal targets, with a focus on minimizing network leakage, building and maintenance of a robust post-acute network, reducing hospital readmissions and decreasing LOS. The position is responsible for providing leadership, interventions, coordination, and facilitation of all activities and services across the system regarding the care continuum. Other duties as assigned. 

Minimum Training and Education: Master’s degree in Healthcare Administration or other relevant field is required. Minimum 10 years of progressive experience in post-acute care leadership. Experience in case management and knowledge of the post-acute care continuum is preferred.

Required Licensure, Certifications, Registrations:  Licensure as a registered nurse, licensure as a social worker or medical director in the state of South Carolina.
